Appointments may be scheduled through our online booking system or by calling our office. By scheduling an appointment, you agree to the following:

You will notify us of any cancellation or need to reschedule at least 24 hours in advance when possible
Repeated no-shows or late cancellations may result in a cancellation fee or discharge from the practice at our discretion
Appointment availability is not guaranteed until confirmed by our office
Roth Family Medicine and Mental Health accepts select insurance plans including Medicare and Blue Cross of Idaho. Insurance acceptance is subject to change. It is your responsibility to verify that we are in-network with your insurance plan prior to your appointment.
You are responsible for any co-pays, deductibles, coinsurance, or balances not covered by your insurance plan. Self-pay rates are available and will be disclosed prior to services being rendered. Payment is due at the time of service unless prior arrangements have been made.
Some services, including certain hormone therapy programs, weight loss programs, and ketamine-assisted therapy, may not be covered by insurance and may be offered on a self-pay basis. Pricing for these services will be provided in advance.
We may offer telehealth visits for appropriate clinical situations in accordance with Idaho state law and applicable regulations. Telehealth services are subject to the same terms as in-person visits. By participating in a telehealth visit, you consent to receiving care through electronic means and acknowledge the limitations of remote evaluation.
Telehealth visits require that you be physically located in the state of Idaho at the time of the visit.
